Request Verification of Contractor's Proper License for Work Being Performed

Contractors working on Los Angeles Unified School District (District) public works projects are required to maintain a California contractor license that is current and active, in good standing and appropriate for the project. Construction work requires a certain type of license verification. This verification is made during an annual prequalification, which is required prior to submittal of a bid. By submitting this form you are aware of a licensing issue or are asking for licensing information of a particular contractor working on a District project.
