Obsolete Certified Payroll Reporting Forms

• LAUSD Public Works Payroll Reporting Form and Notice to Public Entity
  When submitting Certified Payroll Records (CPRs), contractors must submit the completed and signed Notice to Public Entity along with completed pages of the LAUSD Public Works Payroll Reporting Form. This form should only be used on contracts advertised before February 1, 2004.
